Patient Follow-Up

The Community Based Rehabilitation Staff (CBR) routinely visits patients in their homes to check in with patients on the progression of their rehabilitation exercises and use and functionality of their mobility equipment. CBR staff also monitors small grant projects, the integration of children with disabilities into schools and the founding of self-help groups. The CBR visits are intended to continue for at least two years after service to ensure that progress can be monitored and immediate support can be made available if needed.
